Accommodation Grant Form

Every three months, the Staff Welfare Trust and its members award a free week’s holiday accommodation to a deserving nominee.

The accommodation award can be used at any of our wonderful holiday locations throughout New Zealand and is valid for 12 months from date of award.

To be considered or nominate someone please complete the form and tell us why, you / or the person you are nominating deserves the free accommodation. It can be for any reason – such as : dedication to the safety of others, ongoing ill health, great work ethic or anything you can think of that deserves a mention.

This is open to all members regardless of employer or job position held.
